Science Meets Compassion: Diagnostic Innovations are Driving Change in Veterinary Medicine

Did you know that November is National Pet Cancer Awareness Month? To begin a conversation about cancer awareness and the critical role of the general practitioners in its diagnosis, Sue Ettinger, DVM, DACVIM, joins this episode of the Vet Blast Podcast presented by dvm360. Through the episode, Ettinger and our host Adam Christman, DVM, MBA, talk about the role veterinary general practitioners play when it comes to cancer diagnosing, the benefits of building a relationship with the oncologist in your area or hospital, and how important the quality of life is during treatment.

Well, I think often, you know, there's this myth and misconception. And again, that's why I got into social media years ago, was because I knew that pet owners were scared to put their pets through chemo, to torture them, to poison them. And so I always say, and I think about this every time I'm like, approaching a cancer in an individual case in the room, like with treatment, will this pet not only live longer, live well. So you know, when we think about statistics, oncologists love statistics, right? Will the median survival time improve with no treatment, versus X, Y, and Z treatment? But will these pets tolerate treatment? Because I always say, like, if they're living longer, but not living well, I'm not doing my job. Luckily, as you know, most dogs and cats tolerate chemo very well better than people, so they, in general, will tolerate it.

There may be some bad days or some days that they're off, but overall, I want owners to look back and be like, 'Yeah, my pet, still had a great quality of life. We still went on walks... They still watch Netflix with me in the evening.' You know, whatever it is that made Bear Bear, you know, they're still able to do that as they're going through treatment. So again, it's not just improving the survival times or the disease-free interval, or, you know, all the oncology terms that we like, but are they having a good quality of life? Do the good days outweigh the bad days?  Because again, most people... they're scared to give chemo, because I think, you know, they're going to be so sick the entire time, and then they realize maybe there's just a couple of off days as they're going through a chop protocol for chemo. You know, they're much happier.

As this mysterious canine respiratory disease begins to infect dogs in multiple states and make headlines across the nation, many pet parents and veterinary teams are seeking out information to better understand this disease, and how to keep their pets safe. Because there is little information out there, what exactly do veterinary professionals know about this illness, and what can clients do to keep their loved pets happy and healthy?

During this week’s episode of The Vet Blast Podcast, Adam Christman, DVM, MBA, invited Silene St. Bernard, DVM, DACVPM, regional medical director at VCA Hospitals, to discuss the diagnosing process for this disease, how veterinary teams should be talking about it with clients, and how veterinary teams can keep their own pets safe when they return home from the clinic.

: I think that the biggest challenge is that there's a ton of information in the media and we don't have a lot of information to actually provide. So we are seeing an increase in respiratory cases, spotty all around the US and different locations. Some of those cases are cleanly getting diagnosed with assorted viruses and bacteria that we already know about and contest for. But we seem to have some cases in some locations that are progressing to more serious illness, potentially into pneumonia and we don't have diagnoses for all of them. Not everybody diagnoses these for respiratory illness in dogs. It's becoming more common. We've had outbreaks in the past several years. So people I think, are more comfortable with our diagnostics now and will do more testing. But a lot of them potentially get treated just get better, which is what's happening with most of them and don't we don't have a firm diagnosis. 

What we're trying to identify now is, is there something new out there that we need to know about that we either can diagnose and/or need to figure out how to treat. I just don't think we have a clean answer if it's a virus or a bacteria or something else or a combination thereof, a lot of our respiratory diseases are combination disease. We see mycoplasma combined with 

 and things like that, too. So we're trying to do a lot of testing all the veterinary labs, universities, public health departments, on the veterinary side, of course, are all working on trying to come up with some answers. But we don't have one yet.

The specialists that work in general practice always sort of feels like there is this sort of veil between the general practitioner and a specialist and I feel like working toward a relationship and building an intimate relationship with the general practitioner is really valuable. So I want to sort of break down those barriers and make sure that it feels like the radiologist is just somebody down the hall that you can call on as well as the other specialists, but I am going to speak for radiologists and really I want to have that relationship.

So you want to monitor your patient the entire time. Often, if you are going to get that really big anaphylactic type reaction, it's going to happen in generally the first 15 minutes. [It is] kind of doctor dependent, I find more so less doctors pre-medicating with Diphenhydramine or steroids unless it's something that the patient is already receiving for whatever disease process they have going on. But you want to monitor your temperature, heart rate, we do monitor mucous membranes, it can take a little bit longer for those to become pink again, their respiratory rate, their respiratory effort, kind of their mentation.

January is National Blood Donor Month, which brings a focus on blood donation services and needs for veterinary patients. In an interview with 

, Karen Roach, RVT, VTS (ECC), a team trainer at Mount Laurel Animal Hospital in New Jersey, discusses the facility's blood donor program and patient eligibiity to serve as a donor. She also addresses blood types for canine and feline patients including those most common.

The following is transcript of the video:

We have a blood donor program here at Mount Laurel Animal Hospital. We have a lot of regular canine blood donors. So, there is criteria to be a blood donor. The dog has to be between the ages of 2 and 8 [years], healthy, up to date on vaccines. No diseases. Up to date on all the preventives for ticks, fleas, heartworm, and that type of thing. And they have to have the right temperament as well, because they have to lay down and have a giant needle stuck in their jugular vein.

Dogs have—it depends on the source you read—anywhere from 12 to 18 different blood types. We were mostly looking for the negative blood type because that is a universal donor, which is fantastic, but all our donors are typed. All the recipients get typed before they get blood.

Cats actually have to be pretty sedated in order to do a blood draw, so we don't use client pets for that. It has to be staff pets, and we just don't have enough to keep up with the demand. And so it's generally we have to order from blood banks, and it can be really hard to get the blood.

We have the facilities here to also spin down the blood and separate it so that we have packed cells, and then we have the plasma. So the packed cells can be refrigerated for a little over a month, and then the plasma is frozen, and that's good for up to 5 years. And then the plasma itself, if we're just giving plasma that's usually used to treat animals that have clotting disorders and if they have... protein loss and stuff like that. So it's really nice to be able to separate it and only give what we need to have.

Cats have only a couple of blood types. They've got A, they've got B, and they've got AB. A is the most common. When we get a B, we're like, ‘oh dear,’ because it's really hard to find a donor for that.

Understanding metabolic dysfunction

Brennen McKenzie, VMD, MSc, MA, discussed common metabolic changes that are risk factors for disease and declining health in canines, in an interview with 

®. A clinical practitioner, he is also director of veterinary medicine for biotechnology company Loyal, which is developing therapies to extend healthy lifespans in canines.

McKenzie was speaking today about metabolic dysfunction and common mechanisms that lead to aging as a presenter at the 2024 Southwest Veterinary Symposium in Fort Dallas, Texas. The talk also addresses interventions that can help prevent health decline in canines.

A partial transcript of the interview is as follows:

: So, metabolic dysfunction is one of the ways in which aging makes us more vulnerable to disease and disability and health problems and, ultimately, to mortality. And we think it's a particularly important component of aging because it's modifiable. It's something we might be able to change in order to extend health and lifespan.

The basic components of metabolic dysfunction are changes in insulin sensitivity and the levels of insulin in the blood, changes in adipose tissue—how much of it there is, where it's located, the kind of metabolic functions that it's pursuing, and the impact of that on health. And then changes in blood lipids; things like cholesterol and triglycerides and fatty acids. The last one is particularly interesting, because we tend to ignore those in veterinary medicine, our patients don't get atherosclerosis and don't have, MIs and strokes, the way that humans do.

But it turns out that changes in all 3 of those things, in insulin and adipose tissue and blood lipids, are really important risk factors for disease and for health outcomes in older dogs. So metabolic dysfunction is this aging related change in all 3 of those things, and we've done some interesting research connecting those biomarkers to actual clinical outcomes, which is something that's new.
