Client handout: General behavior questionnaire

Have your clients fill out this form very time they visit and track your patients' anxiety from the start.

There's no denying it—pet anxiety causes problems for pet owners. You can help by starting a behavior program at your practice. Click here for a downloadable survey for your clients to fill out.

Be sure to keep the surveys in the pet's medical record so the technician can review them and pass on an accurate assessment to the doctor. "That way when an owner complains that Max is urinating in the house, the veterinarian can flip back through the histories," says Dr. Lisa Radosta, DACVB.
