Laminated educational review sheets for annual wellness exams engage clients and save paper. With National Pet Wellness Month around the corner, it's a great time to spice up your wellness exams.

When clients bring their pets in for an annual exam, our receptionists give them a questionnaire to fill out that provides our team with information about their pets' lifestyle and any prescription diets or medicines. We recently updated this form and made it more visually interesting for clients while still providing the same information to our medical team.

On the new forms, one for dogs and one for cats, we included a picture next to each question. The cute pictures make the wellness-visit review sheets seem more interesting and more fun for clients to fill out.

We laminated at least 25 sheets for each species. Now instead of printing and throwing away several paper questionnaires a day, we use dry erase markers and reuse the laminated forms. So far, we've been using the new annual review sheets for more than three months and haven't had to replace any yet. A bonus: Clients have mentioned how great it is that we're reusing the forms and saving trees.
