Idea Exchange: Patient care form for blood glucose curves

When we admit patients for blood glucose monitoring, we ask clients to fill out this form. Then we know exactly when the pet was last fed and when it received its last insulin injection. This form helps us a lot because sometimes owners misunderstand our directions and are inadvertently noncompliant.
