We had an unfortunate snafu (thermal burn) by a team member. To try to form a silver lining around the dark cloud, we decided to have a meeting in which team members discussed the many things that can go wrong, ranging from dogs escaping to tracheal tears. Stories from experience and hearsay were told to educate everyone. The point was not to make everyone paranoid, but to learn from the mistakes of others. As Eleanor Roosevelt said, "You can't live long enough to make them all yourself."
