Podcast CE: Panoquell-CA1 and its role in the management of acute pancreatitis

A veterinary social worker can play a crucial role within a clinic. Similar to a human social worker, they work within the hospital as a liaison between the client and veterinary professional, providing support during difficult decisions, offering crisis intervention in challenging situations, and more.

 Through their role, they help ensure a hospital runs smoothly and the staff members within its walls feel supported, seen, and heard.

On this episode of the Vet Blast Podcast, host Adam Christman sits down with Stephen Kochis, DVM, chief veterinary officer at Oregon Humane Society (OHS), and Kelly Bremken, MSSW, VSW, the first veterinary social worker at OHS, to discuss the highs and lows that come with being a veterinary social worker, how important veterinary social work can be to a community practice or shelter setting, and what the duo thinks the future of veterinary social work will look like.

What's great for me and I think a lot of the staff is like, you know, Adam, you and I we were trained to be good doctors, we know the medicine [and] we know how to help the pet but we don't always know how to help the person with the issues that are sort of impacting their ability to either take care of the pet or treat the pet or you know, whatever else might be going on in their life.

So, for our team like we can, in these more emotionally charged or issues or where there's cases where there are other factors at play like fleeing domestic violence, [homelessness], substances, whatever it is, Kelly comes in and takes the human part and we focus on the pet and the medical part. Then, you know, it just shares the weight of the whole process across the people who are experts in dealing with all the different aspects of this. Sometimes, Kelly is just a conduit, like a caseworker reaches out to her and says, ‘Hey, I have a patient that needs their pet to get spayed and vaccinated to get into assisted living’ or whatever and then she talks to me, we get it on the schedule, then boom, it's done.

Exploring veterinary social work

The Companion Animal Euthanasia Training Academy (CAETA) trains veterinary professionals and staff in euthanasia best practices. Their coursework includes programs on the physiology of death, sedation and anesthesia before euthanasia, euthanasia techniques, and challenging situations and dysthanasia.

Founder Kathleen Cooney, DVM, MS, CHPV, CCFP, and academy director Stephanie Messick share their experience and insights into the euthanasia process and how clinicians can improve the experience for patients, clients, and themselves.

"What we found is that those who do a lot of euthanasia-related work in their hospitals, and they do it well, actually gravitate towards euthanasia. [They think], ‘I get a chance to sit down and really be involved with these families and get to see that human-animal bond at such a tender time.’" 

"Myself, I got to the point where I was performing 10 euthanasias a day in private practice, in the mobile sector, and I was ready to do it again the next day. So, those of us who are in this field, what are we doing that allows that to happen and how can we share that with veterinary hospitals [so that] we’re not shying away from it. [Instead], we’re saying, ‘Wow, this is actually a really important part of my day and I’m going to feel good about it.’"

In this episode of The Vet Blast Podcast, Dr Adam Christman interviews CAETA founder and director of education—Kathleen Cooney, DVM, MS, CHPV, CCFP—and academy director Stephanie Messick about how the right approach to euthanasia can make all the difference. 

Episode 74: How your euthanasia protocols enhance team well-being

With the vast diagnostic techniques surrounding oncology, veterinary medicine has begun to shift towards developing more personalized patient care, according to Barbara Powers, DVM, veterinary pathologist at Antech Diagnostics headquartered in Fountain Valley, California. Echoing this notion, Barbara Kitchell, DVM, DACVIM, veterinary specialist in oncology at VCA Veterinary Care Animal Hospital and Referral Center in Albuquerque, New Mexico, stated, “We are transitioning now in veterinary medicine, following a personalized medicine approach in humans where it’s not just what the title of the cancer is, but really what made the cancer happen in this individual patient.”

, both Powers and Kitchell joined Adam Christman, DVM, MBA, to chat about the importance of a healthy communication pathway between veterinary oncologists and pathologists and the latest diagnostic techniques and targeted treatment options within the oncology space.

According to Kitchell, the shift has also brought about the idea of discovering if there are any signatures that veterinarians can exploit in making therapy more effective and less toxic for the patients. “It’s just a new threshold for us to cross as clinicians. Having both better immunochemistry and molecular profiling, and all these new things that allow us to understand what the tumor is—is what guides our therapeutic choices to do the best we [can] for individual patient care,” she said.

Adding to that, Powers elucidated the importance of the oncologist and pathologist relationship. “Well, the communication between the oncologist and pathologist is really important because [the pathologist] is looking at a piece of tissue on the slide, and the [oncologist] is looking at the whole animal,” she said.

Leveraging the oncologist and pathologist relationship

One example that Kichell provided where it’s crucial that an oncologist collaborates with a pathologist is in the field of lymphoma care. She explained that this field has morphed into 2 broad categories: B cell and T cell lymphoma. She stated that within those 2 categories, the pathologist can say more about what species of T cell it might be because that “does make a difference in selecting how aggressive the therapy should be, and sometimes even what drugs we should use.”

Another advantage identified by Powers is the pathology field becoming digitalized. She explained that she can now look at slides via the computer, no longer relying solely on a microscope. The plus side of this is that any pathologist can get a second opinion or perspective on tumors they might not be sure about by simply sending the image slide or case to another colleague.

Personalized veterinary medicine has enabled the use of utilizing a pet’s genetic profile to help determine a diagnosis. “We’re looking at genetic alterations [and] gene mutations—that is something that is just starting to be very important now and can help us with the diagnosis even,” remarked Powers. She continued to explain that a certain set of mutations might be more suggestive of some tumors vs another, so when pathologists are stumped with the second opinions and immunochemistry, genetic profiling can be used to help offer clarity to the diagnosis process.

Expanding on this, Kitchell shared that although genomic studies in felines are lagging behind their canine counterpart, this method can help oncologists understand what exactly “went off the rails” in a cell that led to it becoming cancerous and what therapy approaches are most beneficial. “The new molecular medicines in human medicine are vastly exploding. There is probably about 280 individual, small molecular inhibitors, and we’re just getting started in veterinary oncology to apply those tools to our patents, but we're already seeing the benefits,” she imparted.

Both Powers and Kitchell concluded by sharing similar sentiments on the uptick in clients seeking genetic profiling to garner more information about a specific diagnosis. In addition to this, genomic sequencing not only allows clients to become more involved in the diagnostic process but also helps in elevating therapy plans by fostering a more tailored treatment approach. “That’s rewarding in a field like oncology where we have lost so many patients; to have new avenues and new hope of better outcomes is exciting,” Kitchell remarked.

Powers agreed, imparting that one of the greatest advantages is that the biopsy sample that pathologists have—where they’ve made the diagnosis—can also be used for genetic profiling. "The clinician does not have to go back and do another biopsy or take another sample because [pathologists] already have the [genetic profile]. We then send it off to labs that will do the genetic profiling,” she concluded.

Episode 70: Advanced cancer diagnoses lead to more targeted treatments

 as well as an author, consultant, professional speaker, and AKC judge, the inspiration behind her book—

—came after browsing through the veterinary well-being section of the American Veterinary Medical Association (AVMA) website and realizing a majority of the stressors in the field are attributed to people like her, the clients.

“I started looking for a book to recommend to all of my dog show friends because they tend to have the same kind of relationship with their veterinarians that I have with mine and the book didn’t exist," Weaver told Adam Christman, DVM, MBA, in this episode of The Vet Blast Podcast. 

"So I just took what I knew about neuroplasticity and positive psychology and QPR [Question, Persuade, and Refer] suicide prevention training and put it all together into a book. The book is to help clients be much better clients and take that piece of veterinary stress away," she continued.

Apart from her book, Weaver disclosed invaluable advice to veterinary professionals struggling with making their mental health a priority and setting professional boundaries.  

"Don't take misbehavior personally, so that's rule No. 1. Rule No. No. 2 [is] to put a fence around your time off. You have to have downtime, you have to have downtime, you have to have downtime. If you give clients your cellphone number or if you're connected to them on Facebook, put a fence around your time off," she emphasized.

"Rule No. 3 is to give yourself 15 minutes every day for your mind, your body, and your soul. It can be when you first get up in the morning, [and] just as simple as inspirational reading, journaling, meditation, or a quick run-through of some sun salutations and yoga poses," Weaver concluded.
