Idea Exchange: Share this at-home blood glucose monitoring form

If you do a good job training your veterinary clients with diabetic pets, they can do some of their own blood glucose curve monitoring at home. Heres a printable Word doc with the basics.

My idea? At-home blood glucose curve tracking.

This is something I frequently train clients with diabetic pets how to do. What I'm finding is clients tend to get a little confused about when to check blood glucose and how long to continue the curve, so I made this take-home form (Word doc).
