Case 1

Article

Starting treatment with insulin and a dietary change is our best next step for Felicia.  Studies have shown that diabetic cats do best on canned, high-protein, low- carbohydrate diets,1 which are most like their natural carnivorous diet (mice). After starting Felicia on an insulin regimen (2 U of glargine twice daily) and the appropriate diet, she returns for a blood glucose curve about 10 days later.

This is Felicia’s glucose curve; keep in mind that Felicia receives her insulin injections at 8 a.m. and 8 p.m.

Felicia’s physical exam shows that she has lost another 6 oz and her owner feels that there has been no change in her clinical signs.

QUESTION 4

What is your next step?

a) Increase her insulin dose to 3 U b.i.d.

b) Do nothing and see if more time on her current dose helps her clinical signs

c) Switch to a different insulin
